6. Then, type in your first name and your last name.
7. Following this step, you should make sure to mention your country's name.
8. The drop-down menu is displayed. Select the state you want to visit from the drop-down menu.
9. The system will prompt for the Zip Code.
10. In the appropriate box in the appropriate field, type in your full birth date.
11. Choose your football team of choice from the list below.
