It appears that there is an issue refining thunder accessories up to +150 after you have transcended them, using firelord stones.  From what I have heard, this is an issue for both rings & necklaces.  However, I have only seen this problem myself for a firelord thunder ring.  After I transcended my thunder ring, it will not allow me to refine it further through the forge, and it even shows up as shining gold when you view my character (indicating that it is max refine).

Below are pictures of my firelord thunder ring, my forge, and a picture of s.17 Grace, who has a firelord venom ring.  As you can see, Grace's venom ring is working correctly, and he has been able to refine it above +70. (+116 in the picture)
